Medical Documentation Proofreading Example 13

Proofreading

Ms. Brenda is a 69-year-old lady who presented for the hospital with chest pain or respiratory  insufficiency. She had chronic lung disease wit bronchospastic angina. We discovered new T-wave  abnormalities in her EKG. She has undergone a four-vessel bypass surgeries in 2001. We did a  coronary Angiogram. This demonstrated patent vein grafts and patent intern mammary vessel, and  there was no obvious knew disease. She may continue in the future to has angina and she will have  nitroglycerin available for that if needed. Her blood pressure has been elevated. So, instead of  metoprolol, we have started her on Coreg 6.25 mg bid. This should be increased upto 25 mg b.i.d. as  the prefferred anti hypertensive in this ladies case. She is on a ACE inhibitor. I would see her in a  month to six weeks. He is to follow up with Dr. Timothy prior to that.
